Poor guy. I think he'll make sure he doesn't make the same mistake with us, since we are his new boss.
I recall him saying that she mostly locked herself away in her office in act 3, so I guess he have not seen her enough by then to know just had mad she'd got? Plus, it seems like he was in extreme denial surrounding Meredith, he wanted to trust her.

Considering none of the Red Templars are acrobats on super-steroids and we've yet to see anything else magically come to life thanks to the red stuff, I'm prepared to believe that the final battle was Varric exaggerating again. During the development process, they made it seem like something he was supposed to more often than he ended up doing.
I doubt it. I think what we saw Meredith do was very real, not a metaphor. And, on top of that, we have barely even seen what the red Templars can do with that power anyways.

Well we do know that he is extremely loyal so there had to have been some things that have occurred behind closed doors that we don't know about. I mean to get him to the point that he would fight against his commanding officer and likely put his career on the line that he holds so dear to his heart hardly seems to be something he would take lightly. I feel like he would need multiple instances and solid evidence for him to say: okay, she needs to be stopped and I have to be the one to step out of line and tell her this.

Suppose they find a journal that belonged to Meredith...where she discusses using Red Lyrium?
That would be a good way to explain why the red Templars started using the red lyrium!

Hmm, I wonder if the lyrium merely augmented her powers initially, while simultaneously amping up her paranoia the more she drew on its power to enhance her own.

Well that's the thing, we don't know. Maybe the team had decided to perhaps expand on that story and actually have her ingest it in order to show the steps of which red lyrium takes. I mean, I highly doubt that Templars just start drinking it, are totally fine, and then POOF they have crystals all over their skin and are delusional monsters (even some of them can be reasoned with). So, perhaps paranoia and delusion are just some of the first steps.

Yes, and killing Hawke was apparently the line Cullen was not willing to cross. Originally they were going to arrest Hawke but Meredith seemed...bloodthirsty by the end. When I go back and think about it, I find that I can still become confused as to why Cullen decided to protect Hawke, not that I'm complaining. I do believe there were some things going on behind the scenes that made Cullen finally say "enough".
"This is not what the Order stands for." Is he talking about genocide, especially if Hawke is a mage? If Hawke is not a mage, killing the Champion of Kirkwall is going too far? What if they helped the mages? If they helped the Templars, Meredith says Hawke deserves to be punished for harboring a mage. *scratches head* Sometimes I think I have it figured out, then I go back and play and I'm confused again.

I wouldn't want to hear about it really. I'm glad my Amell got together with Alistair. Even if she had a chance to get with Cullen, I would still prefer her with Alistair. Even though I was Amell and I'm now inquisitor, I'll be in an entirely different mindset. I might actually get a little jealous of myself (Amell)

Yeah, then it begs the question: Why Hawke? Why not in the very beginning stand up against his leader? I love that scene with Hawke, don't get me wrong, but it felt like he was exasperated and was more of saying "I've had enough of all of this nonsense and I finally need to do something about it". So, Hawke was just the line being crossed, at least, that's what it seemed like to me. They weren't particularly close at all so it wouldn't make sense for him to have such a soft spot to defend them. Heck, even with Amell, he was ready to kill if told to do so even if he had feelings for them! So there was obviously more that was going on then we know about.

Their mutation whilst on red Lyrium is a gradual thing, prolonged use causes them to mutate over time. It's like what happens to Tervinter magisters that go through years and years of heavy Lyrium abuse, except it happens A LOT quicker on red Lyrium due to it's sheer potency and it's contents.
According to one of the devs or writers (I forget which one) we will encounter varying degrees of red Templar mutation, from none at all if they haven't been using long, to those behemoths that are essentially mindless automatrons, and all that's in between.
