Has it ever been said whether Cullen is his surname or first name? I'm positive it's been mentioned but I just can't remember.

Interestingly, Cullen was a surname derived from an old French form of Cologne. Maybe he's Orlesian afterall lol

It's a first name. And I think it means handsome? Which is very accurate.
