The major concern I have for DAI (and TW3) is in how they'll mix the story-heavy content their games usually have with the open world content. Being not completely OW is a smart thing to do, in my opinion, but we'll have to wait to see the results.
Though I don't understand the problem with your example. It seems very similar to how DAO was, expect the keep part. i guess that if the majority of the quests will be about conquering keeps it might become repetitive, but Indont think it'll be like that. The Emerald Graves and Exalted plains doesn't seem focused on keeps, and we'll have in the game villages (and possibly one-two cities) to focus on. We know that there'll be at least two: Crestwood and Redcliffe.
And Vivienne's recruitment might be around a Circle.
They mixed things up more in DA:O, you had the thieves quests, the throwing dead bodies down the well, dialogue based ones, silly ones like sending the companions in to rescue your Warden and Alistair by having them dress up in stupid clothing, etc. It also helped that we could have conversations anywhere, which broke up the hack 'n' slash nicely.
