So did i at first, but then I looked into the mafia history lately and got interested in medieval Europe, and was surprised just how many analogies to history they are making regards to that time. i think Zevran confuses us to a degree, but yeah Antiva has a mafia, a great merchant business(best in Thedas) In fact the devs admitted the Country would have been called Calabria except that was a real place in italy so they changed it. And two places in Antiva, Treviso and Rialto are actually real places in real Italy.
The characters from Antiva on the other hand, speak spanish and have somewhat spanish accents.

You're prob right about this, I just always assumed it was DA's fantasy version of Spain.

Bio’s worldbuilding team more or less deliberately mixed up things from different eras, countries and languages. Ferelden, for instance, is the setting’s ‘quasi-Anglo- Scandinavian’ monarchy. It combines Icelandic chieftains, the godar (Banns), Anglo-Scandinavian Jarls (‘Arls’) and early to high medieval German territorial Dukes ('Teyrns') with a simplified model of the medieval German elective monarchy.
It’s like making a new car by combining a T-Ford chassis with wooden wheels from the Oseberg chariot and a suspension system created out of Bavarian lederhosen, powered by a modern English Rolls Royce jet engine. But hey, that’s why it’s fantasy

Let's be honest, if Bioware wanted to set a game in a city, they should have chosen Antiva from the start. Kirkwall? What's Kirkwall? Antiva was the city of adventure back in the old DA:O days. They didn't need to invent anything. Hell, you already have Templars and Assassins there
Add some pirates, Tevinter Magisters, Qunari, corrupt merchant princes, Zevran, some royals and mages, and you're sold.

Now my mind's going: what if Dorian is DA's Caesar, and he's going to march an army into Minrathous, be declared an enemy of the state and effectively start a civil war, which will result in him becoming a monarch?Also, monarchies changed over time. Some started elective and became hereditary, then moved back and forth. The Roman empire is a good example of such a shift.

Even though that makes no sense when looking at Dorian's character. But its interesting at the very least. Would be nice to see something of a politicial shift in the Imperium. Say, the fourth DA game deals with a Qunari invasions. At the end of that it leaves Par Vollen in a crippled state. The Imperium does have to worry about its northern borders anymore. But then one of the just-finished war's most succesful Imperial generals stands up and effectively turns Tevinter into a monarchy of sorts. There you go, status quo for the fifth DA game.
Or something like that.

Not far-fetched at all. I've thinking os something similar myself. Because a setting like Tevinter asks for Roman-sized plots. You don't need to invent anything: take a look at real Roman history and you have enough plots for a whole series of games.
From a historical point of view, it would mirror the rise of the Empire over the remnants of the Republic. If it sounds too Roman, there's another Medieval analogy: the rise of authoritarian monarchies that would end up in the birth of the modern nation-state. In fact, Tevinter has some good ingredients for that.
Also, it would make that country more palatable to the players. "Yes, they're evil, but you're going to win the war and change that!".
