Maybe it's because I'm playing Game of Thrones (the Telltale version), but I suspect Otranto wouldn't duel anyone without a strong title or house behind them. Otranto stood to make a name for himself in the North if he bested the rising power of Southern Thedas, so that's plenty impetus to accept the challenge. But if the Inquisitor was just some random horned mercenary, dwarf criminal, or Dalish elf forced into the Inquisition's service? I seriously doubt Otranto would have spent the ship fare to even send a human messenger to say "thanks, but no thanks." He might have sent a bird, though
I suppose that raises the question of whether Josephine would have been so reckless as to become involved a serious romantic relationship (one worth breaking an engagement for) with the Inquisitor if they were non-human and never recieved have a proper title ( seeing as though human quiz starts off as a titled noble). A bit in the episode I just played reminded me how practical nobles must be when it comes to these matters...
The duel scene is undoubtfully more fitting for a human noble (and it was perfect for my champion human noble Quizzy) but the Inquisitor is much more than a Dalish elf, or a Carta Dwarf, or a mercenary. It's not so unthinkable, in my mind, for a noble to accept the challenge of the Herald of Andraste, Lord/Lady Inquisitor of Thedas. It woudn't be dishonorable to be defeated by the Herald of Andraste and it would bring great fame to House Otranto to win that duel. If the Inquisition is powerful enough, it could be necessary for Otranto to accept the challenge, because being rude or showing an open disdain to the Inquisitor could be seeing as an open feud with the Inquisition.
But it's true that the relationship between Josie and the Inquisitor sounds very different for a human noble. Easy mode for them. If the Inquisitor is a human male noble It would be very logical for the Montilyets to talk about marriage after the defeat of Corypheus. You know "Mawiage is what bwings us towethew today..."
