I think that the worst is to have a romance which will end badly.
I don't necessarily mind romances "ending badly" if they're well written.

Also, ending badly is a big YMMV. There are people who think that the Leliana romance ends badly, for example, because she and the Warden are split up by circumstance (even if they are still together). Similarly, people think the Morrigan romance ends badly in DA:O (whereas I think it ends quite beautifully, in a way that really treats her character with respect by making her more than an object the protagonist acquires).

I'm not really that set on monogamy. The only thing that would annoy me about the LI hooking up with someone else is if my reaction was forced. As for them dying, it depends on how. It could be very well written and emotionally moving, or it could be really cheap.

Monogamy (or polyamory) is about communication and consent. Even if you have an open relationship, an LI hooking up with someone else would still be cheating if it doesn't adhere to the rules set out by the couple. The problem with properly portraying that in game is that taking up time with these sorts of arrangements just seems to be too complex to be justified resource wise.

It would probably be best to just avoid infidelity plots, since they can't easily account for a range of reactions by the PC.

No please, Bioware already did that...did both in one game in fact, and it was just ugly.
Moreover, it isn't as though male main characters have ever had any of their LIs given such plots. And they (dude protags/male players) have more LIs in total without nary a one given such a plot. Something special only for the ladies.
