Who's to say they were even in different countries? We know essentially nothing about the Warden's whereabouts during DA2 other than that he/she was apparently going to visit Denerim in act 3.
If the Warden's King/Queen it's kind of unlikely that they've been living in Orlais.

So is pretty much anything else regarding the Warden.
You could end up finishing the game with both romanced without gaming the system, because the confrontation is linked to approval. And there are events and choices throughout the game that can lower companion approval without using the feastday pranks. Also Morrigan doesn't have to be near or at max approval to begin a relationship with the Warden, while she does have to have very high approval to trigger a confrontation over Leliana. That happened to me on my first time through the game and I had no idea they would confront you on it until visiting the forums.
That is gaming the system. You are either not talking to companions or purposefully maintaing very specific levels of affection on each of them. That is metagaming and is using a bug in the system (dev's didn't anticipate people purposefully lowering companions opinion just so they could romance 2 at the same time).

The first time through the game I ended up with that result, going through completely unspoiled and without skipping talking to characters in camp. Morrigan doesn't have to be anywhere near max approval to start a romance and the confrontation where either one of them forces you to choose only occurs if they are both close at 'love' in approval. Additionally approval ratings can drop after starting a romance without the romance being terminated, just by choosing dialogue options or actions those characters don't approve of.
It might not have been intended by the writers but you can end up with that result unintentionally.
Leliana is notorious for having a buggy romance. There were and still multiple bugs within the approval/romance system that can happen involving Leliana. Being able to finish the romance with both Leliana and Morrigan was no more intentional then having her talk to female wardens about how they're marrying alistair instead of being with her even if they never even bothered to talk to her...
Hell my last playthrough I got a bug that made Leliana think we already had sexy time for the first time even though I hadn't so it wanted to skip the whole initiation dialog...
I certainly was not meta-gaming for that outcome, hell I was surprised to stumble upon a new Leliana bug because I thought I knew them all.
I hope DA:I doesn't have any issues like that...

Yeah, you gotta be careful with her conversations on consoles. I had no prior problems with it but then when I ran another play through I couldn't advance her relationship. Turns out you can't gift-rig her approval to early. Also, I didn't no you could restart the romance.

A couple weeks back a link was posted on the BSN to an article written by someone who had some hands on time with DA:I. From what I recall Leliana wasn't mentioned by name, but the article seemed to be referring to her. It touched on the consequences of your decisions, and mentioned that you could send an agent undercover into Redcliffe, but that the she'd end up captured and tortured by rebel mages, and as a result she would develop a more hostile view of mages after her rescue.
Because of the trailer with Leliana being captured by mages, it was assumed she was who the article was referring to. Maybe someone else posting here remembers it and saved the link.
