My Warrior Hawke was a Reaver.
They didn't seem phased by it 
At least Revear!Hawke does this to enemies. Blood Mage!Hawke does it to companions.

Look at the words used to describe the healing ability: devour, consume, insatiable, feat upon, tear into. Then the icon is that of teeth.
You could argue that it's subtext, or that it's meant to be figurative cannibalism, not literal. I'd say it's strongly implied though.
Given that the otherwise conceptually similar Grave Robber spell doesn't use any of that, I'd say you have a point. I just... can't imagine it not looking ridiculous in combat.
