Grunt looks like he really wants to kill something just then... but, he looks that way most of the time.

I think all krogans give you the "you're gonna die" expression. I think you only get this dialogue if Grunt is currently loyal.

Patriarch should have made an appearance in the omega DLC. How about:

 

Shep's trying to shut down the power to the forcefields, Aria and Nyreen are getting swarmed, just about to be over-run, and in charges Patriarch with a bunch of troops and rescues Aria, giving shep enough time to re-route the power without any interrupts. When it's over, Patriarch says to Aria, "You kept me alive once. Now we're even."

He is mentioned in the Citadel DLC. I have no clue what happened to him though, maybe he died during the siege of Omega.

Last two times I played through ME2, I never got the option to talk to Aria about the Patriarch all, and I'm not sure why. I talked to patriarch once, but next time I spoke to Aria there was nothing on the dialogue wheel about him.

Ahh, thank you very much!  ... Come to think of it, I'm pretty sure I ignored those guys in both recent playthroughs.
