 Just finsihed the Arl of Redcliffe quest after sending a Mage into the fade to rescue Connor. I enlisted the help of the circle of Magi, so that Isolde didn't have to sacrifice her life for that of her son. 

However, when I read the finsihed quest text in the codex, it stated that Isolde died to save her son and husband. I had to exit the codex and rush over to check the grateful walking corpse next to her husband.

Is there a seperate text entry dependent upon how you complete the quest or a simple missed codex entry. I had to laugh when I read that.

It's a known issue. It's simply bugged and most people who save them both get that codex entry. Some who sacrifice Isolde get the entry saying they saved them both.

It doesn't make a difference to the story, just the codex, no need to worry.

Another odd bug is, after making Alistair king and becoming princess consort, and with Anora presumably in the tower or dead, I got the epilogue message that Alistair stayed in Denerim with his love for a time making Queen Anora very uncomfortable until being summoned to Weisshaupt Fortress from which he swore to return soon. Er, what?

I got that too. Definitely some screwiness with the epilogues. Hard for me to mind though, considering the breadth and depth of decisions the PC is able to make in this game.

I actually got that too on the PS3 version
