Other than all the people and things you have to murder in order for the story to advance ... Kolgrim. Even the one time I defiled the Sacred Ashes for the achievement and the Reaver unlock, I still walked back and killed him afterwards. (I haven't even killed the High Dragon as often as I've killed Kolgrim . One time I completely forgot to slay it, so the Ashes were preserved and discovered ... and nobody could get to them. Whoops.)
If we're only talking about companions, Loghain by a mile. I've got seven different world states for DAO and he only lived to the end of the game once. Although one of those was death by Archdemon.

You can have him suicide the archdemon instead of you or Alistair if you don't like/trust Morrigan. That's a pretty damn good reason to keep him around.

I used the Ogre in Darkspawn Chronicles to pick up and punch Alistair to death after he broke up with me after the Landsmeet. I reloaded over and over again until my bloodlust was sated.

Loghain, for sure. Every time but one, (for the "Recruiter" achivement.)
Behlen, almost as often. I was ambivalent about who I should select/allow to rule Orzammar until I played the Dwarf Noble origin. After that, he dies every time.
