I'm here to let you guys know about a new game that is in creation right now that allows players to have full control of the game! This game is like a combination of NWN and UO! While I can't agree that this is the "first", Shards online is certainly worthy of attention from the Neverwinter Nights community.
I highly recommend visiting the link posted above and checking them out.
