That's an interesting question.
Mechanically, Bianca will probably be stronger.
Lore wise, we'll probably have a quest about it. Maybe the lyrium stone is starting to deplete its energy, putting Bianca and Varric in danger? Maybe it's starting to control him?
Only Bioware knows.

Clearly giving him the stone will lead him to be ever more in love and fixated on Bianca.
There can only be one conclusion. Varric will be denied as a love interest if you gave him the stone.

Probably not much, to be honest. It's a small and very optional thing to give him the lyrium shard. I doubt it would have any affect and is either going to be stolen or otherwise overlooked in the coming game. The most that might happen is perhaps one of Varric's friends, crazed by the "song" of the red lyrium, is going to be drawn to him due to the shard. Otherwise, I wouldn't expect much.
Not an issue for me though. I gave the shard to Sandal to turn into an enchantment for my weapon. ![]()

Based on what we know the red lyrium does and the red templars, it would make sense if you allowing to give Varric the red lyrium had some serious issues in DAI. But...somehow I doubt it. It will probably be ignored, I think.

Hopefully Varric managed to have the bit of red lyrium potion and a spell was developed that grants resistance to the influence of red lyrium and can alleviate some of the side affects allowing him to begin having his brother treated. This could later be used to develop a spell that is extremely harmful to those under the influence of lyrium while posing no threat to anyone else.

As long as he doesn't pull a stump-sized Anders I'm cool with whatever happens.
Chances are it will be hand-waved due to his popularity or a minor variation in dialogue options. It would be neat if he was beginning to show signs of decay. I mean, you lose your brother and witness all the shenanigans Hawke got into, mage-templar war included, and with the Inquisition coming you'd start find yourself losing your mind. Add a red lyrium idol and....

I'm really hoping the Red Lyrium decision and the Bartrand one adds to the story of Varric in Dragon Age Inquisition. It would disappoint me if they're just ignored, since Varric is the one telling Hawke's story to Cassandra, makes me believe that Varric was always planned to be in DA:I.
I would like to see a cured Bartrand make an appearance in Dragon Age: Inquisition.

behemoth varric.... it will be the size of a normal person though cause hes a dwarf
although i dont expect it to be too effective on him, dwarves are semi-resistant to lyrium and its side effects

Varric only used the shard to improve Bianca, a weapon that he admits he is obsessed with. Due to the size of the shard, I doubt its effects would be all that noticeable. It would only make him slightly more attached to Bianca than he already is.

Wrong. He was searching for his brother. Who betrayed him and left him to die. He didn't get the red lyrium until much later, after he found his brother.
It would be kind of hard to the red lyrium to influence him before he even has it.

I'm of two minds on this subject. On the one hand, I have a feeling that it can only lead to sadness and tears if Varric keeps the shard. On the other, I perhaps wonder if it's a fake-out...if having the shard allows you to track the Red Templars easier or gain access to a specific area.

He's already had the shard for 3 years by the time Cassandra debriefs him and he seems like good old Varric. Perhaps some red templars will hunt him for the shard? Although that's thin. I don't think that anyone knows Varric has it and I doubt that just because you are affected by it you can magically track it. I think it will probably be a trinket. But well, it gave way too much attack speed to let him keep it. ^^
