At the moment I'm doing another playthrough for ME3. I'm sure I saved all of the crew, but so far I haven't seen Kelly on the Citadel.
Is this a bug?
Kelly not there?

You have to have had dinner with her and gotten her to offer to feed your fish in ME2 in order for her to appear in 3.

As far as I know she only appears if you had dinner with her in ME2.

I'm on my yet another ME2 playthrough and it's the last time I'm trying to keep Kelly alive (without romance). I don't know what I'm doing wrong, I'm nice, I talk to her constantly, yet I never got an offer for a friendly dinner

You can check out this vid (first 3 minutes). It shows taking a paragon option after Thane's recruitment (no invite) followed by the middle option (dinner invite). This is for a femshep. Not sure it applies to an mshep. For mshep you may get the dinner invite after Samara's mission. (I don't remember since it has been a while since I have done an mshep.)

You could always check the docks to see if she's there just in case. But as others mentioned, I believe you have to have dinner with Kelly.

Thx, will pay special attention to Thane and Samara recruitements then. Do I need to have my fish bought (and died at least once already) before dinner?

I recall getting the invite after the Collector Ship mission. When she talks about how worried she was for SHepard, choose the "You do care" explore option
