Does Shepard have to be involved no, does the decisions he/she made have to be yes.
Only if the game takes place in the Milky Way.

I don't see them creating a new gen trilogy while they are working on their current game. Porting will definitely be much more different on their current platform.
They will probably use a keep mechanic

Bioware doesn't HAVE to do anything. They could simply choose a canon ending.
If endings are ported, I'd expect it to be done in the way that DAO and DA2 saves are going to be imported into DAI: via an online service similar to the Dragon Age Keep, except it would be for Mass Effect.
The galaxy map did not look like the Milky Way, so either we are in a smaller sector of the galaxy and limited to that region, or we are in a different galaxy. Or perhaps the scope is even huger, and we're able to travel between more than one galaxy in the new game.
I would also suggest there's no reason to expect another trilogy, or anything after the original trilogy other than standalone games. While you could argue they've learned some things about how to handle returning protagonists, I think Bioware is becoming increasingly inclined to not have protagonists return because of all the potential complications. You'll still be able to carry forward your past decisions, of course.

We don't know exactly how the new game connects to the trilogy, so we don't know how much of the decisions made are going to have an impact. Even if they do have an impact, a solution like the Keep or a more elaborate Genesis comic would be used. At the end of the day, I'd much rather Bioware work on a new game. Also, if there was going to be a new-gen port, it would've been announced at E3.

Very unlikely this would happen unless outsourced to another developer.
I would definitely love to see it happen but I'm not getting my hopes up for it.

Bioware operates under EA, there's no reason why another EA developer can't pick up the project.
They wouldn't need to add new content. (it would be nice if they added some "inbetween" cutscenes explaining what Shep was up to before and after ME2)

Just be careful, because new textures are a game of chance. Like most mods, sometimes they work great, sometimes they do not.
