Oh my god help help help! PANIC ATTACK medal:

Bronze : Use a Missile to kill the first Atlas, Prime, or Banshee you see.

Silver : Use a missile to kill a lone Banshee, Atlas or Prime
with one bar of armor left.

Gold : Scream on the mike for someone to missile a lone Atlas, Banshee or Prime when you run out of missiles.

Don't panic - Run the entire length of the map, under fire, with no shields and your health dropping fast, only to die in hard cover from an across the map impossibru luckyass husk shot.

 

Slight variation when you die from the DoT effect of a banshee warp ball that was aimed for your ally.
