what do you actually get when you beat the game on Hard/insanity?

It would unlock gamer pics (Saren and N7) for use on your gamer profile on the 360, if I remember correctly.

you get bonuses that go towards your skills for getting certain achievements including completing the game on certain difficulty achievements.

Also if you unlock the achievements you get a percentage boost to experienced gained for future playthoughs. 10% for Hard and 5% on top for Insanity.

actually its 10% for getting the power gamer and 5% for getting the extreme power gamer.
