I'll go with "Systematically determined based upon which NPC is deemed the favourite by the player" because I'm a monster 
Well you would base it on time spent in the party which would either force people to spend a lot of time with a hated character, or just accept the inevitable. Or just force them to sacrifice their LI, that always goes down well.
