If the first time you go outside Kirkwall with Fenris is during The Long Patrol on The Wounded Coast. The Slavers will spawn but no cutscene, mention or quest will show up. After that the Slavers will NOT spawn ever again in game thus you must reload to an earlier save as other quests that have cutscenes in the same area don't play well with his companion quest trigger.
Bitter Pill will not trigger...

Fenris' ambush will trigger after you've completed Blackpowder Courtesy for the Qunari. Have you done that? Do you get the "ambushed!" loading screen when travelling to the Wounded Coast?

A Bitter Pill can be done at any time in act 2, Blackpowder Courtesy is not required.

Dutchess is wrong.
Sometimes the ambush only gets triggered if you travel to Sundermount. It sounds like it's triggering for you but without a cut-scene.
