For the dialogue where he says he made a mistake by trusting his friends and he mentions carving every angel from her face, I believe it isn't actually referring to an individual. I think he is just giving the female gender to that mistake. I'll look to see if I can find the actual quote, but for me, I got the impression he was calling trusting his friends a mistake a she the same way one might call a country or a ship or justice a she.

This is exactly how I read it when during my first playthrough.

@Nightwolf-I feel the same way! I can take my Inquisitor being left because Solas thinks he has to do this on his own or because he wants to protect her, and I don't care if he's had a past (which I'm sure he has given a certain line.....But I'd hate to find out he's called another "his heart". I think at that point, I'd just completely stop romancing him in my following playthroughs because the depth of his words would almost lose their weight.
