Fiona left the Grey Wardens and became the Grand Enchanter at some point. I think she was part of the Monstimmard Circle at one point but don't know if she was ever the First Enchanter there.
You have to be a First Enchanter to be a Grand Enchanter but her being First Enchanter at Montsimmard is just an assumption based on the fact that this was the circle she was originally part of.

Viv's VA in the demo wasn't french and we don't know where Viv is originally from. She's just described as Orlesian. We also don't know if everyone from Orlais will have a french accent or not.

Yes but it doesn't mean that is where she is origionally from or that it will be the accent she has. Leliana identifies as Ferelden for example.

I agree that Grand Enchanters are appointed from First Enchanter's but I'm not sure if Fiona was a special case. I skimmed through Asunder and couldn't find anything specific.
I'm also not sure if Grand Enchanter's give up their position as First Enchanter of Circle X. Fiona could have been First Enchanter of Montsimmard and Vivienne took over after Fiona was promoted to Grand Enchanter. This might be something Vivienne will talk about in game.

According to the Wiki it seems like they live in Cumberland so I guess that would be evidence against them keeping their old title.

Yeah, that's what's throwing me off as well. I think the 'next in line' info comes from the Game Informer article from last September. They could be mistaken or misheard something the devs said about Vivienne.

Antiva itself is supposed to be similar to a medieval city-state like Venice, but Antivans speak with Spanish accents and even use Spanish dialect, including Zevran and Feynriel's father. For example, Zevran tells his love interest, "Si amor?", while Vincento exclaims, "Maldición" in Kirkwall.
I've actually crafted my previous two canon protagonists as ethnically Antivan (although they are culturally Fereldan), and I plan to do the same with my canon Inquisitor.
