Vivienne doesn't strike me as prim and proper. She seems more like she throws her weight around. Not in as...masculine or brutish a way as Cassandra, but definitely not demure as the phrase "prim and proper" implies.
That said, she may indeed, and in fact probably does, have a secret. Don't they all? Don't we all?

Like all companions in Bioware games, each character will likely have issues and problems of their own. I have no doubt that Vivienne has her own problems, whether she keeps them secret or not.
I can't remember the last companion we had who was happy with the way their life was with no ambitions or regrets.
