I still find it kind of weird though that there are no deductions for all of the people Balak killed.

That would penalize people who did not have the DLC.

I think having situations where the outcome of a major event depends on multiple smaller decisions made before would be a good idea.

In the Next ME I would like more choices like Kaidan or Ashley, Kill Wrex or Save Wrex. I want characters to be offended by some of the choices my PC is making, like in Origins. More timed missions that if you don't do them in a certain time frame a character may die. It adds to the Realism (? if that's even the correct term) and makes it more engaging for the player and actually have consquences.
I also wouldn't be opposed for the story to be on a smaller scale, like ME1. I mean yes you were trying to keep Saren from reaching the conduit to end all life, but it's not ME3 where every planet is falling to the Reapers and Shepard is in Purgatory getting drunk. It makes it hard to enjoy what the Galaxy has to offer. Like Miranda said in the Citadel DLC.
I really don't want color coding, it needs to be more complex and for a RPG like Mass Effect it would be nice to have small decisions make a big difference.
