This might not actually be a glitch, but I'm trying to understand why a party member isn't able to hit enemies more frequently.

My current understanding of AC/THAC0 is this:

(Player) THAC0 - (Enemy) AC = minimum dice roll to hit (out of 20). Anything less than this number is a miss and anything greater is a hit.

So I'm wondering if the spell "Holy Power" is working properly. After Viconia casts it (and Righteous Magic, for a big strength boost), her THAC0 goes down to an impressive -5 (base 3). I'd expect her to be hitting almost everyone consistently with a THAC0 like that, but in reality she's missing fairly regularly. I'm testing her against some Pirates in Brynnlaw (wearing just leather armor), and she only seems to be hitting them about 50% of the time. I can't imagine these pirates have an AC of less than -5, so does anyone know what's going on here? Any experience with Holy Power not working? Is there a console command that I can use to check the pirates' AC? Can I check to see that Viconia's THAC0 is actually being used by the game's engine?

I have Throne of Bhaal installed and while I do recall installing a couple of patches, I can't remember the details. If that would be useful information, I'll try to track them down, or if it's likely to solve the issue, I might just reinstall everything (as long as I don't lose my save files... Will I?).

Thanks in advance for any help and this is my first post so apologies if this is the wrong place to ask or something like that.

Not every "swing" (melee attack animation) made by a creature actually translates to an attack roll in game, the reason for this is that those classes (all non-warriors) or monsters normally allowed only 1 base attack per round would otherwise just seem to stand around doing nothing rather than fighting for most of the combat round.

Go to game play options -> feedback and tick "to-hit rolls", this way you will see in the combat log when a character actually non-critically misses their target.

Ah, that must be it. Thanks for that. I feel silly asking such a basic question. Obviously, the reason she didn't seem to hit any more often after casting that spell was because she was already hitting pretty much every time anyway.
