I found Insanity fairly easy, but if you're doing a New Game and leveling up from scratch, just be a bit cautious and use your squadmates effectively.
Also depends on what class you are playing.

The best way to play on the hardest difficulty is with the lights off, and bad 80s music blazing in the background. I mean real bad 80's music.
On a serious note, choose squadmates that compliment Shepard's powers and (especially in a mission with rachni) someone who can take a hit. I find Liara completely useless, so don't bother with her.
Detonations do higher damage at higher difficulties, so don't be afraid to use them. Javik/Kaidan is a good biotic team for those.
Don't be afraid of Cryo Blast, you can eliminate any non-shielded foe for a while so you can take care of someone else.
I did it on Vanguard and Soldier, and Vanguard was more fun, but I felt that way on lower difficulties too.

Pfft. Lancer is better, plus it comes with a cool DLC.
But the real secret is to give your squad mates Typhoons. (Actually, I stopped doing that since they were not giving me a chance to kill anything myself.)

If your relying on tech or biotics carry relatively light weapons so you get short cooldowns.
You might find part way through the game you're actually looking for an extra challenge. Then I'd recommend using hotkeyed powers and trying to pause the gameplay as little as possible. Using kinect so you don't have to pause also works and gives the added danger of your squad mishearing you.

Sidenote, if you do the Multiplayer much and get used to the difficulty there, Insanity's a cakewalk regardless of what class or setup you have.
Barring that:
Soldier for Adrenaline Rush with Energy Drain as a bonus power for staying power in fights out of cover to shoot longer.
Infiltrator for Cloak damage bonuses with weapons, Sniper Rifles in particular, though I prefer Shotguns and Assault Rifles.
Vanguard just dunks things with Charge, Nova and a good shotgun.
Engineer's a tech combo-fest but not as strong as the previous three.
Sentinel's the beefiest beeftank you'll ever see. Can get a build where you take 0 damage while in cover, otherwise you can use Warp Ammo, slap Warp on an armored target and dunk it with a good weapon and overload shields/barriers to warp other stuff.
Adept works wonders with biotic combos and is the one class that works the absolute best with Liara, Javik and Kaidan due to how much biotic stuff you can throw at things.
It all boils down to what you wanna do. My preference is Vanguard, Soldier, Sentinel and Infiltrator in that order.

I won't deny the Engineer is good. But I found, for me, the Sentinel was the easiest. Combined with Liara and/or Javik I could get off combos so fast it was incredible. Liara's Singularity has something like a 3 second cool down, so I can continuously Singularity>Throw. For an Atlas Overload (Shepard) > Warp (Liara) > Throw (Shepard). Her Warp can be gotten down to around 6 seconds. Tali worked well on the Geth missions and against Cat 6 with her 6 second cool down on Energy Drain. Energy Drain > Overload. I also liked having EDI around with area Incenerate > Chain Overload which can get you two fire explosions. (I love fire explosions.) Plus 50% damage reduction means more freedom to move out in the open, although the Engineer could take Fortification as a bonus power for 40% damage reduction.
Another thing I love about the Sentinel is the ease of comboing with any of my team mates.The Engineer works better with the tech/combat team members. While Liara's Warp can detonate either Overload or Incinerate, her Singularity is basically reduced to crowd control unless you have another biotic for her to combo with. And Javik really doesn't seem to have a way to detonate or prime tech combos. (I may be mistaken on that.)

Javik cannot prime tech combos, but he can detonate them.
His Lift Grenade and Slam can detonate all combos, but the enemy has to be on health to detonate.
But that being said, if the enemy is on health and you are Sentinel, then his Pull is better so you can alternate between Liara's Singularity and his Pull while you spam Throw.

Insanity is easy in ME3. Whatever class will do. If you're on PC I recommend downloading Spectre Difficulty mod, as it's the true way that ME3 insanity should have been.

The thing about Vanguards is they suck when it does not matter, but are the best when it does.
By matter I mean confused close up fights where everyone is out flanking each other, in other words in situations where you might acutally die.
Who cares how good you are at long distance exchanges of fire where you can always go back undercover for a few minutes to recover.

I think soldier may be the easiest if only because it is a forgiving class. I love to use incindary ammo and give my squadmates cyro ammo. I use concussive shots when enemies are primed by fire or ice, causes some nice explosions. This class is also great with a sniper rifle if you like Adrenaline Rush, extra damage. I used Fortification because it looks cool and adds damage protection. Pretty good health and shields and cover is your best friend, you won't have much trouble at all. Techy squadmates help prime explosions for your concussive shots.
