Is it possible to not unlock companion quests?

I am unable to ask Sten any further questions and have yet to get his companion quest. I've already completed red cliff and mages tower (headed to the urn of sacred ashes)

Is it possible to miss a quest because I did respond properly in a conversation?

What's Sten's approval rating? If it's not high enough, that's the problem. You also have a while to go before you should be worried.

He's below 50% I do know that much.

Everyone else I can still ask questions to so I was worried I may have missed it. Because I was reading on the wiki and I've already talked to him about why he was locked up and why he cannot return. But I don't recall having an option to help him find his sword

Check if his approval is marked as "warm", if not, you need to keep increasing it. To unlock his dialogue about the sword, Sten will start talking about how "you're not as callow as he thought" and that's when you know the option was unlocked.

Sure enough.... His approval just wasn't high enough yet. Thank you! I thought for sure from the other conversations I missed out. Thanks everyone
