Is simply haven't the the achievements for each romance enough to unlock this one?
First Knight + Witch Gone Wild + Easy Lover + Wine, Woman and Song = Hopelessly Romantic?

Mine unlocked at the same time as Easy Lover, so I'd say yes.

That's how it worked for me (on PC). I had the Alistair, Leliana, and Zevran achievements forever and ever, but not the Morrigan one. (I don't play male characters, so... yeah.) Then I did some compatibility testing for another modder, using a hack to let a female warden sleep with Morrigan, and viola! Two new achievements that I didn't really want or earn (Witch Gone Wild and Hopelessly Romantic).
