Were difficulty achievements available for DA II? I thought there was a problem with the on-the-fly difficulty slider that made that impossible.
I'll go for the HARD achievement, but not the NIGHTMARE one.
The only parts that did were the GoA and WH DLCs.

Nope. Neither DA2 nor DA:O had difficulty achievements. At least not ones that required a full playthrough of the story.
As a completionist, I have no choice but to accept the challenge. Maker be with me.
One hopes FF isn't the same as in DA:II.
That or enjoy Iron Bull one-shotting your other companions.

This the same list filled with recycled achievement art from previous games?
Just like the game, the achievement list would be incomplete. Placeholder images would be expected. 
But then, it could easily be fake. We'll just have to wait and see.

They actually confirmed the level cap a few weeks ago in the Raptor Q&A session.
I expect it will be raised depending on whether the game gets an Awakening-sized expansion or some such.
