I think the Inquisition has already been formed, and that the protagonist becomes the leader after the cataclysm that takes place. It seems like the soldiers at the end of Dragon Age II may have been members of the Inquisition, since Gaider previously stated they weren't wearing Seeker uniforms.

I think they were (ex) Seeker of Truth, and a group of handful that still loyal to divine justinia v. However the symbol on both Seeker and Inquisition's uniform are similar.

Something has been bugging me lately - I remember reading somewhere that the Breach occurs during a peace talk between heads of the mage rebellion, and the heads of the chantry, and the Breach kills everyone present, save for the Inquisitor. So, would among those dead include Divine Justinia? As, I also read that the Inqusition was her back up plan in the event of a calamity within the Chantry. If it is her idea, and she is killed by the Breach, who informs the Inquisitor of her idea?
Leliana = Left hand of the Divine and Cassandra = Right hand of the Divine.

Which means if Divine Justinia is possessed by a greater pride demon and Leliana + Cassandra reports the Inquisition to her - our Inquisitor will be 2 steps behind the entire game ![]()

It's sad Justinia will die this early in the game but here's hoping we'll be able to converse with her along with other major figures in the Peace Conference.

This, although you don't start off being the leader.. i think we'll end up being forced to join and then made leader shortly after that.

I dont remember where I read it, but there was a conspiracy theory that Divine Justinia in Orlais was possessed by a greater pride demon. Cant remember the details though and its probably wrong.

As we all know, during the peace conference between Chantry and Mages, the tragedy leaves you the only survivor. Both the leaders of the Chantry and mages were present. I read in an interview that the Chantry immediately closed ranks to solve it's own crisis of finding a leader. If I find it again, I'll toss it up here.
WAIT GOT IT!
So when we form the inquisition in this game it's a Master Stroke almost by going through a loophole, because at the beginning of the game when the head of the Chantry and the head of the Mage's faction they're going into the Temple of Sacred Ashes to this peace talk, and they're all killed. There's this massive explosion and this breach in the sky opening up, and the Chantry's in absolute chaos. It's headless. You've got infighting between different factions, you've got people saying screw it let's all retreat back to Val Royeaux and elect a new leader et cetera. -Cameron Lee (http://www.ausgamers...es/read/3432915)

I'm not convinced that everyone is dead, just that thousands were killed.
There may be legitimate plot reasons why specific individuals were not present at that moment.
We don't know for sure do we?
We've had people who were clearly killed make miraculous reappearances.

It is pretty much still just rumors that the Divine dies early in the game. But they are rumors with a lot of credence, so don't hold yor breath in hopes of Justinia's survival. Remember that she is still just a very minor character in the games themselves, never seen and only mentioned, so only the ones who've read the novels has any sort of "relation" to the character. To the ones who only plays the game, she is still just the faceless leader of the Chantry.

We actually meet her during the DLC "Leliana's Song" (she was still mother Dorothea at that moment), learning a bit about her past and that she had previously interacted with the bards of Orlais. Considering this and her clever and mostly sensible and tactful demeanor in the books - she tends to be perceptive for the material world's problems, acting in more than one front with both subtleness and precision - it is very unlikely she would send someone to represent her instead of going there herself. The only window I see for her survival is if she had sent a very competent double to the meeting. Her death seems more likely, though, since it would have a powerful impact on both Cassandra and Leliana.

But her importance to the DA World is major. She's the leader of the biggest religion.

As TKS said, in-world importance does not equal a deep role in the games. Players of the previous game already know that the Divine is an important figure. They don't need to meet her to understand that. New players just need to be told that she is important and they will get it.

To be fair, though, she is/was a very progressive Andrastean, as far as Andrasteans go.
I'm assuming she pulls a Padme and sends a body double, but if she actually dies I'll be upset, apostate though I am.
Although I do wonder why Cameron Lee would let something so definitive slip so early when so many other major details are being kept secret. Seems just a bit too important to be used to build hype, so maybe there's more to it than what we're seeing at the moment.
But if not, as has been mentioned, it would certainly do a number on Cassandra and Leliana, and explain why Cassandra's on such a short fuse a lot of the time, and bitter towards the Inquisitor. I probably would be too, were I her, if I was reminded of the explosion that killed my old boss every time I looked at my new boss.

I think the peace conference plot point was leaked back in 2012, anyway.

Cassandra has a short fuse since forever. She was like that in Dawn of the Seeker.
