I never intend any characters to die on purpose but my during my first play through I made mistakes...
ME1 - Ashley and Captain Kirahe

ME2 -
Legion (forgot to upgrade Normandy sheilds, destroyed from Normandy explosion during suicide mission) 
Tali (shot in the head in suicide mission because of losing Miranda's loyalty

Results were a disaster in ME3 when siding with geth/quarians. Legion kept saying "we are not legion":( and quarian replacing Tali shot herself in the head.

Having Admiral Xen on the dreadnought instead of Tali gives new dialogue from Xen. I liked it. Yes its Admiral Raan that kills herself
 

ME3 -
Miranda (killed by kai leng)
by the time I was ready for the final mission I knew everyone was going to die to I restarted a better playthrough much later.

She will always die in ME3 if not loyal

Even with EVERYONE alive, the ME3 Normandy felt lonely without Kelly Chambers and Rupert Gardner.
 
The void I feel when walked through the now-trashed-up ship (courtesy of the Alliance) reminds me of this music, by Hans Zimmer.

Here's another one I did.

 

ME1

 

Ashley kills Wrex

Kirrahe and Ashley die

 

ME2

 

recruit all squadmates

Jack, Legion and Thane killed because no upgrades to ship

Garrus killed closing the door

Grunt killed escorting the crew

Samara carried away by seeker swarms

Zaeed killed as 2nd fireteam leader

Miranda and Jacob killed fighting human reaper

Mordin, Tali and Kasumi killed holding the line

Shepard dies

 

ME3 never happens. The cycle continues

 

 

Is this really possible? Can Shepard really die at the end of ME 2? I'm going to look for video now.

I sometimes skip Thane's recruitement.

I love Thane's recruitment mission. The reason being is because it is the only mission I can still mess up on. In all other missions, it is impossible to fail. But in Thane's mission, I have sometimes taken a wrong turn and failed the mission. I like that because it brings a little bit of up uncertainty to the game.

Good thread by the way. I was always reluctant to complete the game with missing squad members. But this changes my mind somewhat.

It's also a good point that saving everyone lengthens the playthrough. Letting some die may shorten the playrhrough and give slightly different feel to the game.
