Myself, I've never killed my main on the ballistae, but sure came close to it the first time I played it. Don't stand in front of the big cannon!

First encounter with Flemeth, she grabbed and spit out the Warden dead. Definitely unheroic, and quite a lot of cursing at Bioware for an "impossible fight." Of course, came back with Shale instead and sat back to watch the show.

Glyph of Paralysis combo has nailed me more often than I care to count, but somehow always had the good luck not to have more than one enemy spared by the explosion.

Worst idiotic mistake is probably the Ruined Temple; I go up the stairs, clear out the rooms on the left. Pop back out into the main hall, one mage and three-four reavers. Must kill the mage first, I decide, and have my Sword and Shield bravely charge down the mage. Unfortunately, after several casts of lightning and what not my warrior is dead before he can reach her.

Funnily, I still won the battle; Morrigan casts Waking Nightmare to confuse the mage, getting her killed by the reavers, while Wynne revives my fallen Warden, and with the mage down, it's about Cone of Cold and Glyphs. Lesson taken: Mages rock. Warriors do not.

Let's see...my first run-through I decided to test out earthquake during one of the sections in the Ruined Temple. Only the room was awfully small, my characters had very poor physical resistance, and the enemies had archers standing by. The fight went something like this:

Cast Earthquake.

Fall down.

Get impaled by lots of arrows.

Your Journey Ends...

the high dragon killed me and alistair as we were both warriors. i thought being gobbled up and thrown up was pretty great, despite the fact i had to finish the fight with arrows.

Used tainted blade in the Loghain duel, finished the fight with <10 health, and died back in Eamon's estate the instant the dialogues ended.

I shot Alistair in th eback with one of the balistas. That's when I realized they could kill me. yes, did shoot myself once with one, same fight...archdemon. You just have to activate it and then run in front of it (toward archdemon). If you're fast enough, you shoot yourself. Bummer.

Tried out tainted blade during a fight. Fight went down to the wire. Forgot to turn it off.

At the Tower -

1. Aha! A ballista! I'll use it to kill all those darkspawn...(fires ballista and loses 3/4 of health)

At Lothering (almost a death but still humorous) -

My party got overwhelmed and killed by the wolves and then I ran around lothering being chased by wolves for what seemd like hours with everyone ignoring me and the wolves ignoring everyone else. People just don't want to get involved these days.

I dunno, but I laughed at the fact that after taking on a ton of darkspawn, cleaning out the back alleys of denerim, and taking on countless undead in redcliff that my party was first taken out by a pack of wolves that were smart enough to laydown about 20 traps!

Once in Lothering I got the dark spawn to chase me and run toward the bandits to see if the would fight each other. They just ganged up on me.

 Seconded. But worst. You cannot even trust your companions. Not a death, but still.... Anyway:

On the Korcari wilds, fetching those damn treaties. Next to the battle with the emissary, there is a hidden chasind cache. Rogue darkspawn... spawn there. I entered with my dalish elf on a Conan style (two blades, not a big one though)... only to find out that my companions were at the bridge happily watching the fishes.

You can see Conan (so to speak) running away from the battle with four genlocks at his feet towards his companions. Yes, he survived. Ironically neither Alistair nor Ser Jory (the instigator, for sure. Damn coward....) survived the battle. Only Daveth and I (with aid of potions, of course).

Just learned a new trick with two mages, a rouge and the sten. Encountered a party of genlocks and hurlocks. Put a glyph of repulsion on one of the mages and the other mage is casting a glyph paralysis on a genlock that is closing in fast - new codex entry - what the ...?! - Then watching my party getting torn to pieces by the rest of the genlocks and hurlocks

That would be two mages, a rogue - not a rouge, but a rogue. One is make up and is applied to the face - usually the cheeks or lips to give them an attractive bloom of colour. The other - isn't.

And yes, paralysis explosion is fun - unless you cast it on yourself. :o

A case of perfectly timed quotes here. I was in Denerim, can't remember who I had in my team but I definately had Zevran. I was running about and got a random event in one of the back alleys. I snuck in and ended up with everyone in my team full of arrows. Zevran's final words? "I haven't felt this much pain in" - is filled with arrows and dies - "well, ever."
Also when I redid the fight, everyone but Zevran died again and I thought I was going to own them. I said, "All the arrows are missing him!" About a second later, Zevran was filled with arrows again and dead. ¬.¬
