I have seen the second slide only once, but it was quite some time ago. On subsequent playthroughs I was thinking that it was part of being all renegade, and having Samara talk about how she might have to kill Shepard in the future. But so far I haven't been able to find what triggers that slide.

Nah, I don't think it is tied to renegade. My of my last 3 characters 2 ended up with a 40/60 Paragon/renegade rate and one was all renegade. All of them got the line from Samara and none the slide.
(But that could also be a bug, I got the same Tuchaka slides while Wrex alive or dead)

I'n getting ready to start a ME3 playthrough in that I purposely failed Samara's loyalty mission. I did it by asking Morinth about Justicars. She didn't like that. So maybe doing that will trigger that slide at the end of the game. Time will tell.

I don't think that's it by itself. I have at least one playthrough where I failed the LM because Morinth got up and left, and Samara still showed up at the monastery and was still seen with Falere in the epilogue.
