I think his head is too small as well but I don't have the photoshop skills to show an alternative. His horns are bloody huge.

Personally I think Iron Bull's head is too large as it is. If I had my way, it'd just be horns on a neck F**k it, just disembodied horns.

I've always thought his head looked to small, but those alterations in the OP definitely make it too big.
In the first image, the original looks like his head is smaller than Sera's. In the altered one, it's bigger. The most I could support is making it the same size as Sera's, methinks.

I'd agree with this except that we aren't comparing humans and "Qunari" are naturally larger than humans. Comparatively, an average sized "Qunari" would still be bigger than a large human.
But Qunari don't have disproportionately large heads compared to humans. Their proportions are the same as humans, the only difference is average height. Two people with human proportions (which DA humans and Qunari both have) of very different heights don't have a huge difference in head size.

The OP went a little overboard in the second and third pictures, but look:
His head is actually much smaller than Blackwall's in the Before picture. That can't be normal.
Hm... maybe it's an optical illusion because his head is in the middle of those huge horns and body? Something's not right. His head looks very tiny in that photo.
He's probably looking up in that picture. Hence the substantial difference in head size.

His head is a similar size to everyone else's. It's his massive body that makes it look small. What you're asking would require them to shrink his muscular frame, and his muscular frame is one of the defining aspects of his physical appearance.

When real people get extremely muscular, their heads don't grow, also the difference in size between the heads of someone 5' tall and someone 7' tall is minor.
Technically, this is only partially true. Very tall people generally do have larger heads than smaller people--the growth of your skull is in proportion with the growth of the rest of you as determined by hormones. Some people's bodies grow out of proportion (although the difference is usually slight). Now, if you take an OTHERWISE average-sized person and bulk them up, yeah, their head won't get bigger. But the Iron Bull isn't exactly "average-sized" for a human. Artists tend to study "perfect" human proportions, which are fake averages that don't actually represent the myriad differences that can be present in actual people.
The Iron Bull's head doesn't bother me because he's a unique model, unlike the giraffe necks, overly long legs, and insanely outsize hands on ALL the females in Origins. That annoyed the CRAP out of me. Just relax, realize that the unique characters don't all have to conform to some kind of Platonic Ideal of "correct" proportions, and move on to bitching about the weird distortions on the general models.

If you get a closer look at him (difficult) you'll notice that his skull is anchored on his neck slightly differently from the way a standard human's would be. He has that in common with the DA2 Qunari--their head is forward on their neck instead of on top. It makes the Iron Bull's head look smaller due to the angle induced and the fact that he doesn't have the huge beak nose of the DA2 Qunari. His jaw also has a sharper angle than the more squared-off jaws of the DA2 Qunari, which accounts for a lot of the difference.

I meant the size difference in heads of different sized people isn't extreme, not that it doesn't exist.

I think his face has been a bit flat and blurry in the demo videos but the smaller size doesn't trouble me any. And the photoshop 'fix' you suggested seems too far in the other direction. Plus iron bull isn't a human and they have already changed the Qunari twice in the last 2 games, elves too.
Maybe just me but I always liked the Rand character design from the Breath of Fire series too.

lol I always thought Bull's head was too small and oddly placed on his body, ever since the E3 trailer from 2013. But these photoshop efforts... ahh... erm... no comment xD

There are a few factors, I think, one is that his neck is pretty large, but that makes sense. The human neck is barely adequate in supporting the weight of the head/brain, now if we added a few more pounds for horns, the neck would have to be more muscular.
Another is his shoulder/arm muscles are so pronounced they make him look broader shouldered, affecting how we read his proportions, but if you imagine where his bones would be, his proportions seem less weird.
Next is posture: he's pictured standing in his classic Iron Bull bro pose. Arms crossed, standing up straight, with his head raised. Since most characters are shorter than him, dramatically so, the "camera" has to be at an angle, dramatically distorting Bull's proportions so that he seems larger at his "base" (arms, shoulders, waist) than his head, which is further away and at an angle.

I have seen some guys that have a very large amount of muscle for their overall body size (they're bodybuilders) and their heads have a tendency to look small in comparison to all the muscle.
