You are separated from your LI by Inquisition duties, and having killed the big bad, excited to come home, only to find that your LI stepped on a rusty nail and died.
Inquisitor has evil identical twin and the whole Inquisition thing was a big distraction from the real threat to be announced in DA4.

A unexplained magic rock drives the big bad insane and gives them mario jumping abilities and the power to bring stautes to life. All dispite the person in question never having any magic abilities.

Just before you manage to close the last Fade Rift a man with a chainsaw hand is sucked through. He will then be the protagonist of the next game in the series: Dragon Age: Evil Dead. Oh and Bruce Campbell absolutely will be doing the voice work.
But that would be the greatest twist ever!

After discovering that they're videogame characters, the Inquisitor travels into our reality hoping that the defeat of EA's top brass would end the exploitation of their world.
Star Ocean 3, positively the worst plot twist ever.
The Inquisitor has to merge bodies with a High Dragon for the final boss fight which in aerial combat fought alone in a combat engine unfamiliar to the player and with counter-productive controls. Also the big bad will turn out to be your father, Zevran's brother Mervan.

The "Elder One" is really a disgruntled elf stripped of his identity named Jon, whose going to trap the Inquisitor and then steal your fade powered soul for himself!
The Iron Bull will of course betray you to him. But he was placed under a geas by Jon before you meet, so it's not really his fault for betraying you.
You will travel to the fade and defeat him, regaining your soul, and in the end ruin his ambitions to become a god.

I liked it. Not to bring ME into this but the Geth/Quarian peace is the spit in the face of the Star brat and its flawed logic.

Reading this thread is fun. I someone's joke will be correct. I know people hate a story that has been done before but I honestly don't know of a story that isn't like another.
Every story and Cliché has been done. Its all in how you tell the story that catches peoples eyes.

I don't know if I can necessarily come up with a 'stupid' plot twist - as someone else put it, it's all in the way it's executed.
However, a plot twist I wouldn't like to see: It turns out your Warden was killed off screen, and the big bad is walking around wearing their skull as a mask. (kudos if you get the reference lol)
Or really just any variation of the Warden/Hawke getting killed off without player input. :\
Also would hate for it to be revealed that the Maker is really a thing. I rather like the whole mystery surrounding all of the Thedas religions, as to their validity. It adds a lot to the atmosphere of the world... Though it could also be because I like entertaining the idea that they're all dead wrong. XD

The Inquisitor's been dead (or possessed [depending how it's done]) the whole time.
Hawke and the Warden died so the Inquisitor would have his power.
16,777,216 different endings. Any combination of 256 shades of green, 256 shades of blue, and 256 shades of red.
Time travel.
Maybe a couple involving Thedas and its relationship with the rest of the world...

1) You must tame and befriend a Ferelden Frostback. In turn, she teaches you the finer points of debating in dragon language, where yelling is key. The big bad is an ancient and clever dragon (old god) but you can win the game by "shouting" him to death.
2) The big bad actually arrived via The Crater world map encounter in DAI, and is a very powerful alien. The only way to kill it is by obtaining the Starfang sword the Warden had made out of the Starmetal that was found in the crater. Alas, it either failed to import from your DAO save or alternatively, some jackass lodged the sword in a huge boulder. Only if Iron Bull is in your party do you have the option to retrieve the sword and complete the story.

You find out that the Inquisitor is actually the "Superbeing" although no one believes this or wants to believe this. See, all of the nobility of Thedas have been in a secret Chantry breeding program for years. They have been trying to produce the Superbeing who will be able to go into the Fade and untaint it. The only reason you survived the Tear is because your mother defied orders and bred outside the program producing you. You will raise the Inquisition and go on a mighty Crusade conquering Thedas before you seal the Rifts and cleanse the Fade. How can this be? Because you ARE the Kwisatz Haderach!
