Got one more to add: Never sided with the Baroness.

DAO:
Loghain spared
Alistair throws a tantrum and leaves the Wardens or he's executed, depends on my Warden's personality.
DA2:
Anders gets intimate with Murder Knife.

I never
left Sten to die
annulled the tower in Broken Circle
wiped out either the werewolves or the Dalish
told Zerlinda to leave the baby in the Deep Roads (very minor, but you never know...)
preserve the Anvil of the Void
allowed the burning of Amaranthine
spared the Architect
allowed Nathaniel to be killed
sided with Petrice
killed Genitivi
(OK, I'll admit it I never killed Alistair. Even on my playthrough with Anora ruling alone and GW Loghain, I turned Al into a drunk. People do sober up sometimes.)

Wow. That's a lot. I actually enjoyed the burning of Amaranthine. It was a very Loghain-esque moment for my canon Warden. He'd officially fallen off the hero wagon. The Architect could go either way with me. I let him walk in my canon because all of the companions with me at the time thought it was a good idea.

Be nice to the elves or the qunari, I tend to RP very prejudice human mages. It makes for some interesting dialogue.

These things I always have to do:
Kill Connor
Wipe out Redcliffe
Execute Alistair
Kill Dog
Side with the reavers and kill Wynne and Leliana at the Urn
Because I'm just such a good person.
Edit: OK, I might have once done the opposite of all those things...

I never:
- make a deal with the demon possessing Connor (sometimes I save him, sometimes I'm too lazy, but then feel really bad about it in the epilogue)
- side with the Templars during Broken Circle
- side with the elves in Brecilian Forest (I think I always lift the curse)
- side with Harrowmont
- side with Branka
- desecrate the Ashes
- spare Loghain
- let Anora rule
- spare Architect
* play aggressive Hawke
* take the sibling to the Deep Roads
* agree with Merrill (she is the only companion who is always in rivalry with me)
* give Fenris to Danarius
* give Isabela to Qunari
I've made other choices regarding some of the above situations, but just to see the outcome and reload. There is one things I have never done even for the sake of variety: I have never sided with the Templars in the end of DA II.

I must recruit every person willing to join. That includes companions, advisers, people like that Avvar in the demo, merchents, builders, everyone.
Of course there's the problem of mutually exclusive people.
