Never accepted Vaugh's bribe at the end of the City Elf origin. In addition to that, I never let the guy live to get his vote in the Landsmeet.
In Awakening, I've never killed the Architect. Everything else in both games I've done at least once.

Never killed Loghain, and never killed Anders. People with pragmatic conviction and the will to see their convictions to the end are rare. Loghain needed to be taught he was wrong, and he was. Killing him doesn't bring back anyone at Ostagar, and 'justice' doesn't help you fight the Archdemon.
Anders is completely right in that there is no way to compromise the circle as it was, and only a total reboot was going to give mages any semblance of bargaining power over their lot in life. 'Reforms' would never work, because there were too many people invested in the status quo.

I don't buy many games as there aren't that much that interest me these days....or the ones that do are made by developers with no standards that release the most broken piece of crap games that you can't even enjoy because of problem after problem. DA: I will literally be the only new game I have bought this year. 5-6 years from now I will have played through the game at least 5 times and gotten my monies worth.

I like to get as much for my money as possible.
As for things that TK World Constants:
- Varric is always Hawke's BFF. I just can't bring myself to rival him.
- Aveline is always Hawke's second BFF, and is always with Donnic.
- Anders is always rivaled. I friended him on my very first DA2 playthrough, and that was the last time I made that mistake.
- High Dragons are always dead.
Can't think of anything else that isn't fair game.

DAO:
Always romance Leliana. I did Morrigan first but can't bring myself to do that again.
Loghain is always executed for his crimes.
Anora is always married to either myself or to Alistair.
DA2:
Always play Sarcastic Hawke. He's just too damned entertaining.
Always rivals with Anders and Merrill, BFFs with everyone else.
Always get Aveline and Donnic together.
Always do the one-night-stand with Isabella. I like how it comes up in party banter later.
Always romance Merrill. Can't help myself. That accent...
Always rivalmance Merrill. I did friendmance first, but rivalmance is so much better that I can't go back.
Anders always dies. At that point in the story, he's a monster.
Never side with Meredith. Same reason.

As for the thread question...
There are certainly things I haven't done, and feel no desire to, but it's always been more of a reflection of my lack of interest in replaying DAO than anything I can't stomach in a RP perspective. I certainly have created characters in the past who might, say, accept the Tevinter blood sacrifice power up in DAO, but none of my Dragon Age characters did.
Now, I do admit I felt far more restricted playing Shepards, but that was because I view 'professional soldier' as a sort of limiting factor for any Shepard that didn't have some other handwave for why they wouldn't fall into that spectrum. Which existed... but I've rarely seen 'personality altered by prothean beacon/Lazarus project' as a rational for Shepard characterizations.

There is only one thing in the dragon age games I will never do. Have the warden as Alistiar's mistress. ick, that's a no for me.
edited, because I was wrong. there is more. lol I cannot hurt the dog. Zevran always lives (he's my favorite character) Fenris will never be a slave again.
Other than that there might be things that I generally do in most playthrough s, but my current Aeducan is going to be making all the decisions I have done very infrequently, or I have never done before. It should be interesting.

I never kill Wynne, she reminds me too much of my grandmother (she was also a teacher and used to nag all the time too lol). I just couldn't do it. Even in my evil playthroughs I recruit her last after defiling the Urn.

Some decisions are more pragmatic and require sacrifice. But some decisions are just evil. Most of these I tend to classify as more evil options.
