It's the Inquisitors shadow, their true self.

Question is, why is the Twinquisitor hellbent on destroying the world?
Dun dun dun.
As mentioned in Awakening and in DA2, linear time as mortals experience it doesn't really exist in the Fade, so maybe whatever part of the Inquisitor was left behind ended up going completely doolally? If after being trapped in limbo for what felt like eternity, only to discover that I was never really "me" at all, I'd be pretty angry too?
(Although I doubt they're repeat the same storyline that's already present with Cole?)
Another possibility is that if the Inquisitor's glowing hand is because they've been marked by chaos, maybe the Twinquisitor is a manifestation of that chaos within the Fade, furious that the Inquisitor's mooching off their power and decided to take their form to kick their teeth in?
That ain't really the clone's fault. That's more on Brooks/Rasa for not even having the slightest idea as to what kind of a person Shepard was.
Clone Shepard's problem is the same reason the Illusive Man was so adamant that they bring the real one back from the dead, because without any of the history and struggles that made Shepard the person they were, they'd never have that same unique spark that defined them.
Like Grunt, they were simply shown the cliffnotes of Shepard's life without any context and had no reason to care about any of it. Grunt was lucky however as he was created as a unique entity, whereas Clone Shep was told from day one that they were not only just a copy, but a copy of someone who was a legendary hero throughout the galaxy, causing them to develop an massive inferiority complex as a response.
It also didn't help that the person who was responsible for his mental and emotional development was Brooks, an obvious sociopath.

Well; if the voice is a demon, than the "you were a mistake" bit could actually be about "all" mortals, hence the images of elves etc....while its talking.
That everything made after the spirits of the fade was a step back and something that should not have been.

Maybe the Inquisitor is the clone.
It would be pretty cool to play a villains clone in a game.

Well, looks like I'm gonna have to break out my “it’s the real me” phrase.
