Josephine. She is a diplomat and hopefully unbiased so she will be most effective in dealing with the various groups, within and without the Inquisition, and allies that the Inquisitor will encounter. So all of the behind the scenes stuff she can deal with and anything that involves going out into the field that is of course the Inquisitor and Companions job.
Well, Spy'y stuff, soldier stuff and politic stuff is obvious.

I'd pick Vivienne since she is already high in court.
I'd pick Cassandra as my first choice for field leadership, Blackwall would be a second choice and then Cullen.
Cassandra and cole would probably be the big commanders. But in combat probably iron bull or cassandra.
Who would you trust as your first lieutenant? It can be of the companions, advisers, or any of the other NPCs we know about.
I honestly don't know the companions and advisers enough to make that decision. If we're including characters who we knew about in the past, my elven Inquisitor would pick Velanna, since she was trained to be a leader and changed for the better (and presumably defended a human village on her own given how she didn't perish at the battle for Vigil's Keep and wasn't manipulated by her ghoul sister), and my dwarven Inquisitor would select Sigrun, given her experience in fighting with both the Legion of the Dead and the Amaranthine Wardens (as, presumably, she's lead units of her own after the Mother's demise).
as a field commander, i would definitely put cassandra in charge (although i will mostly take her with me). and if i can't, its probably gonna be iron bull if its straight up combat, but if it has something to do with magic or the elder one or something that would be outside the normal combat lines, then it would be dorian.
i would also pick cassandra. she is strong, loyal, a good leader prob, cares about protecting people and first and foremost wants the truth with no bullshit attached.
Who makes sure that order is kept at Skyhold and that the daily routines are maintained? Who's going to make sure that ale is refilled at the fort tavern?

More seriously, if you had to separate your companions into two groups (ala Mass Effect 2), who would you put in charge of Team Bravo?

It depends on tasks of leader though. If it's order and battles then Cassandra unless she's with me when Cullen is in charge. If it's more like daily life of Keep I think Josephine will be good to keep everything organized and running smoothly. If it's similar situation as Suicide mission in ME2 then I'd choose someone with military experience as well.. probaply Cass or Iron Bull if she's not available.
Are we even able to make a second in command? And what I am more interested in is how the Inquisitor exactly gets put into leadership. Gaider said in a youtube interview that the Inquisitor isn't put into the role of leader right away so I wonder what happens that puts you in the role.
Well, Dorian will be my second in command. when I go out, I want mage to control MaheSkyhold for me BUT in fact it's nearly impossible becasuse i will bring him with me all the time
So, the one who fit the duty will be another warrior : Cullen or Cassandra
But right now I'm barely know them. I will reconsider again when I have a chance to know them more. Who know, maybe some minor character in Skyhold can surprise me.
Probably Dorian or Solas, I feel I can trust them to not be just terrible.
