The first time I played through ME3, I was so excited just to finally be playing through the last part of the trilogy, that I really didn't pay attention to how bad some of the flaws in the game were. But once I saw the ending, everything else just started to stand out, and by the time I managed to bring myself to do another play through, everything I didn't pay attention to the first time bugged the crap out of me.

My biggest ones were the retcons of me2 squad mates, the overly pro-geth stance the game had, and everything with Kai leng.

So what are the biggest flaws that you didn't notice until after the ending?

I think I experienced every flaw in ME3 before the ending. When it got to the ending, I fell asleep and woke up in 2013.

I loved Rannoch the first time I played it.  Now, it gets a little more unbearable every time I play it.

 

They butchered Legion, it's overtly pro-Geth (or at least pro-Pinnochios masquerading as Geth), they butchered the admiralty board, there is very little backstory or debate over the Geth/Quarian situation, and they made reaper code the answer.

 

The gameplay there is still pretty fun though.

That's difficult to answer. Some of the flaws were very visible before the ending, like mostly everything associated with Cerberus -- because the game continuously flaunts you with it. I guess after watching the ending it turned some of those parts about the game that were a little weird or awkward into bad once you find out how they're resolved. For example, the way the Crucible was built up I knew it was going to be a trap because of it's convenience, unexplained nature, and how half the game focuses else where despite it being, "our one hope to stop the Reapers." In the ending though it basically turns out to be the weapon (energy source?) it was stated to be which brought attention to it's nonsensical design history and construction.

That the game really wanted me to get with Liara  for some reason. So I did a new run to see if it made more sense that way and it did.

That I wasn't as disappointed as I expected to be, given what I'd already heard about the backlash to the ending by the time I got there myself.

The flashbacks when shooting the tube. I wonder why I had Anderson, Joker and Liara. After a couple more playthroughs, I went online and found out I wasn't the only one that got Larry, Curly and Moe. Unless you romanced Ashley, Liara or Kaidan, it would always show Liara. Fortunately, the extended cut fixed that.

If you didn't activate Legion in ME2, or if he died, that damn VI really makes killing the Geth seem reasonable. The way I see it, Shepard frequently determines the light in which you will see ongoing issues with past actions. If you decide a Geth is too dangerous to activate aboard your ship, or not worth going out of your way to assist in a loyalty mission, then they will prove to be dangerous and not worth helping later. If you choose the renegade options for dialogue and decisions consistently these things end up proving themselves. If you were paragon with Legion in ME2 but renegade in ME3, or vice versa, you will seem like an ass. That's real flaw there, your inconsistent role playing can deprive the game of sensibility. Or maybe that's just your choices mattering?

I did not like that my fuel tanks were suddenly downgraded to 1,000. I am almost certain that every other upgrade was kept in the SR-2. Why did the Alliance feel the need to reduce the fuel capacity by 500 units? What was the purpose of the drive-core retrofits? Also, why did they not put speakers back into the Normandy? I miss that ambient music that would play in the SR-1. It made every conversation seem more dramatic than it actually was. Also, why was there no space battle scene for when the Reapers took the Citadel? What happened to everyone on it? Who made it out alive? What happened to the Citadel Fleet protecting it?

The biggest thing that confused me the second time around was stopping and thinking 'wait, the citadel can move? How?' And then the reapers shut off every relay.. Except the one to earth... How lucky.

That didn't happen. Don't confuse a clumsy endgame- gate for an in-universe event.

In all other cycles, when the reapers take control of the citadel, first thing they do is shut the relays off to isolate all systems. Getting past whatever reason as to why they didn't take the citadel first like every other time, did they just not bother when they finally DID get the citadel? Considering at no other point in any of the games do we get no choice in which system to travel too, no matter the situation, it seems stupid to do it now no matter whether this got said in the game or not. Either way there's questions
