I was told that i needed to beat the game on insanity in order to get the picture. I beat the game all the way through on insanity and got the achievement..but sadly no picture. Was i misinformed or did i do something wrong? If i need to do something else to get the picture please let me know.

You mean this:
Oh boy, if it is... wheew...it's gonna take at least 200 hours or some serious grinding challenges of Multiplayer to get that. I'm over 500hrs already, and I still haven't gotten it yet. 24/25 challenges currently until I get it, but I'm too lazy to promote.

That gamer picture is when you complete Mass Effect 1 on Insanity.
