I name my characters by just having a huge list of names somewhere and pick out something that I want. Either that or I just pick something out from whatever fandom I may have.

I'm kind of good at thinking up names for mmo identities (my forum name is actually an acronym of different online identities I've used in the past), But when it comes to actual names I kind of fail.
If I can use the default or get away with something I'd use in an mmo, I use that. But if I can't or I hate the default name, that's a problem.
Usually what I do is take a cool sounding word and start changing letters one by one till I find the sweet spot. If that doesn't work, I just use the name of a character I like. Don't have too much patience for that. Named my second DAO character Nihlus for example.

Warden: Urania Amell (mage). I used Urania for all my female heroes for quite some time, even my Shepard. Urania is the Greek muse of astronomy and astrology.
Wilhelmina Hawke (mage). A character from Dracula, very Victorian. Just like the name.
Inquisitor: Samantha Trevelyan (mage). Because Alix Wilton Regan.

I'm sort of a wild card; sometimes I try to be lore friendly and other times I like to make names that fit with the character's look as well. If you see my icon it's the character concept I have for my elfmagequisitor as well. I'm naming him Albedo because of his hair color, not really lore friendly but I like the sound of it.

It depends. I do like to reference some of my favorite characters/stories/games/mythology when naming my PC.
For example:
Lucien Hawke -> Lucien Lachance (TES: Oblivion)
Ebon Hawke -> Ebon Hawk (KotOR)
Tamriel Aeducan -> Tamriel, Nirn (Elder Scrolls)
Sirius Mahariel -> Sirius Black (Harry Potter)
Aries Hawke -> Aries, God of War (Mythology)
Elphie Hawke -> Elphaba "Elphie" Thropp (Wicked)
Though sometimes I will just pull a name out of my anus. I have one Amell named Death and a Brosca named Memphis.

I research like crazy and test out the names before I really play. Learn their meanings and such. Seeing if the name match my characters

I try to come up with unique names for those characters I plan to be my "main" ones. Apart from that, the most important consideration is that they sound good when spoken. The DAI surnames are limiting in that, because they all appear to be stressed on the second syllable, which means that one-syllable first names break a rhythm I've become fond of. While I usually know the sources and the meanings of pre-existing names or name patterns, they usually don't influence my choice.
If there are existing naming conventions I feel I should adhere to, I may also use existing names from the setting. And there's one other hard rule: I never, ever use names of well-known figures in famous stories, unless they're also common names on Thedas. I don't want my characters to be associated with other fictional worlds.
Some of my names:
Eorlin (Amell), my main Warden:
Chosen for the sound and because it sounds somewhat Old English, so should work as a Fereldan name.
Rowan (Hawke), my main Hawke:
Obviously a common Fereldan name, also the name of a famous figure in Fereldan history.
Lilaeya (Levallen), one of my planned Inquisitors:
One of the few really unique names I ever came up with, this is one I used before in one game about 14 years ago. Chosen for the elf Inquisitor for the sound, the rhythm and alliteration it makes with the surname, which also has the advantage of further weakening any older associations I may have left in my mind.
Maelyn (Trevelyan), one of my planned Inquisitors:
Created from a naming pattern I often use, characterized by approximately the same vowel combination in two-syllable first names (or the first two syllables in three-syllable names) for females, which I probably got attached to because of Rhaenys Targaryen (a dead character in A Song of Ice and Fire).
Marada (Aeducan), one of my Wardens:
Taken from a comic by John Bolton, a pale, white-haired female warrior. I made a dwarf who looked similar by accident in DAO's CC and kept her. I also found the name sounds cool. My favorite Warden after Eorlin.
Velian(ne) (?) (Adaar) [WIP], one of my planned Inquisitors:
This illustrates the kind of trouble I keep running into when making names. It all started with my wish to have a decisively non-qunlat name for this qunari who hates the Qun. Everything that entered my mind was another instantiation of the abovementioned naming pattern, and I didn't want that. Others meant something in an existing Thedosian language ("Dalenne" is too similar to "Da'len") or something I didn't want associated ("Ira" is Latin for "wrath"). I imagined a Rivaini backstory but that didn't help since there are no naming conventions for Rivain and Rivaini names are all over the place.
I eventually tried to vary "Veliss" (which is, yet again, made from the naming pattern). "Velian" is another two-syllable stressed on the first and I find that boring, so I tried the variant "Velianne". However, that sounds not at all good with "Adaar" unless you put a particle in between the names or add another syllable somewhere. So now I'm looking for something that could be used as a particle in a meaningful way together with "Adaar".

I also had a Katrigan Aeducan that may be a second choice as my favorite Warden.
I choose them very carefully depending on their origin, personnality and family, but I don't bother about the meaning. This time I think I'll go for Astriel Lavellan but I've not made my mind already.

For other games I used to pick names from the Silmarillon. For my Hawkes I looked for celtic names using this french website, and will use it too if I go for a human inquisitor. For the elven names I found some nice names in this generator, the problem is I don't know their meanings, if there is one. If I play a qunari, I'll go for a word from their vocabulary that I like the sound of and the meaning, and I haven't searched for the dwarves yet ...

hmm, I try to think of forest things for elfs, like ivy, thorn, willow. dwarfs, rocks, ruby , coal , humans nobles , i have been using game of thrones names, cersi, jamie ,bran. Sometimes i use names that im called in game.

For my Wardens it's just whatever I feel like at the moment of their creation.
For my Hawkes I put up two criteria:
1 - It must begin with 'A' to match Bethany and Carver (ABC, as soon as I noticed this my brain wouldn't let me ignore it).
2 - It must be complicated or embarrassing enough to explain why everyone just calls them Hawke.
My favorite name for a male Hawke is 'Aristide', as it's a family name and fulfills both criteria.

I used to name all my characters virtue names (Patience, Solace, Diligence, etc.) but since they are embodiments of spirits of the Fade in the DA universe, I think I might have to come up with another method. So for my first playthrough as a female Dalish rogue, I might go with Irish legends and name her either Emer or Niamh. That might go better with the game lore. Dwarves would be easier, as I could go with gems and stones for names. Humans could be anything, I guess, but Qunari will be tough.
