Maybe try to focus on something else when those fights break out? If you switch to casual you might be able just to click to keep attacking enemies and press some abilities and beat the enemies you have problem with while scrolling tumblr or watching something from youtube. If they aren't boss fights at least.
I seem to remember a mod for Origins which replaced the spiders with Mabari for the arachnaphobes among us. Maybe something similar will be doable for those who can't live with giant crab monster things.

I have (or rather, had) a real phobia about heights, so bad that I would get all the symptoms (dizziness, sweating palms, nausea etc) even in games. When my then employer moved to a glass fronted multi-storey building and sat me at a desk on the 4th floor beside a window, I walked out on the first day. They had to send me to a therapist to help me get over it. She was very good, and used a lot of stuff from Assassin's Creed (which I had told her I loved but was finding difficult to play because of the climbing tall buildings thing!) to help me get over it. Or at least get over it enough to be able to work in that building (and play AC!).
@people who don't understand what kind of phobia this is:
It's not about fighting the spider. It's about looking at holes. Yes, it's completely irrational, but so are most phobias.
Even an innocent background in "the Longest Journey" is disgusting to look at.
every phobia has a trigger,i mean you are not born with it,for me it's no different,when i was little i had a accident involving hot coking oil,the skin does not react well to that (still have the scars)
Why not see a professional? Going to see a therapist sounds like the only way to manage your phobia. I know some people avoid therapists because of an imaginary stigma associated with seeking mental help "I'm not crazy so I am not going" sort of mentality. But you would see a doctor if your arm was broken, you would go to an optometrist if you needed glasses and you would go to a dentist if your teeth needed work. Why not see a therapists for your phobia?
