I would say the first playthrough with Cassandra, the second one with Josephine.
Also, I'd hate to interject, but women who are not straight are more fetishized, not more respected than not straight men. In media as a whole, gay women are likely to be treated as a punchline or some weird male gaze sort of fantasy and bisexual women are usually treated as relationship/marital aids. Pansexual and asexual women exist as punchlines or not at all.

Putting my own bias aside, I think this is a good point... Vivienne is a woman who uses words and cunning to get what she wants, whereas Cassandra clearly favors force. Josephine is a born diplomat, so she may have more in common with Vivienne...

I can never get into a game where the main character is a woman. Too much of a disconnect for me. Sera seems just fine if she wasn't exclusively lesbian.
If I'm being perfectly honest, I've never really understood this argument. But it's so common among male gamers that I'm not going to pass it off as an utterly invalid excuse.
I think, to a large extent, it depends on how many games featuring female protagonists you've allowed yourself to play. If all you've ever played is games with male main characters, then it probably won't be easy for you to just jump into another game with a female character. However, there's also nothing stopping you from giving that game a try. Once, twice, little by little and you'll see yourself getting used to the idea of playing a woman as the hero.
At least that's how I think I got there. I don't honestly remember a time when I had issues playing as a woman. I played the original Resident Evil games some 15 years ago without any problems, and they all featured female heroes.

Some people, I guess, can't break out of their comfort zone to try new things. Hell - my dad is nearly 50, straight as an arrow. And has created both male and female characters. And romanced Leliana, Zevran, Fenris, *And* Anders on various playthroughs through DA. Even done M!Shep/Kaidan and M!Shep/Cortez.
Some people just have more ability to walk out of their comfort zone. I'm a man, Bi - Male Preference, and create female characters to try and enhance my gaming experience. Some of my favorite games are where there's a Female Protagonist. Perfect Dark (Zero) to name one.

As for playing cross gender....
The PC is not you and can't be you, unless you have magic Fade powers and grew up in a medieval society. You're creating a character much like a writer. Think if who you can tell an interesting story with.

You may have a point. I actually can't remember a game that I've played that featured a female protagonist outside of Tomb Raider... and I've played many, many games over the years. If fighting games count, then I guess I used to pick female fighers more in the past, but I still favored Ryu and Ken over Sakura (although she was unnaturally strong in the early Street Fighter Alpha games...). In the original Resident Evil, I normally chose Chris, although I remember at least one Jill run... Tbh I think it's just a preference that some people have and some don't. It's hard to say...
I know most of my playthroughs will be a female Inquisitor (my first one will romance Cullen), but I will play male characters from time to time. I plan to create a couple for Josephine and Cassandra.
But, most importantly, I plan to create characters for the different origins, my decisions to correspond with my other decisions, and the male LIs.
I actually feel kinda guilty for not planning to play s/s or cross-gender.

I often choose female characters for the same reason, but the most important reason for me, usually, has to do with a sense of refreshment. The male to female ratio isn't exactly small when it comes to game protagonists, and I generally want to expose myself to that kind of fresh experience as much as possible.
I actually enjoyed playing Mass Effect as a FemShep a LOT more than I did as a DudeShep.

I wouldn't say fighting games (or sports games in general) really count; other than that, you're right in saying it's a preference. But I don't believe it's one that can't be overcome. Many female gamers play games featuring male protagonists and enjoy them as much as their male counterparts do. I think it's very much a matter of flexibility on the player's part.

I've played RPGs with male protagonists for most of my life. My favorite game of all time, Xenogears, has a male protagonist, so clearly I'm not going to turn down a game for that alone. However, I've almost always rolled as female in Bioware's games (I have one unfinished male elven ranger playthrough from NWN 1) for no other reason than that the narratives from the female perspective are what I'm interested in. I am not uncomfortable playing a male in these games I am disinterested; big difference. I am happy that there is representation for female heroes and I'm going to enjoy it.
It's entirely possible that I may roll a male PC down the line if I become interested in doing so.
Most of my Wardens were female, all my Hawkes are female, I will also have many female Inquisitors. Only had two male wardens, I may have one or two male Inquisitors, definitely one for Cassie, not sure of the second yet. He may ride the Bull or get stuck to a stache if I play him.
