After I killed the second harvester, I leveled up.

I went to the squad menu and pressed the button to view the "right" squadmate. I pressed it again, and there was no squad shown and only one "locked power". I pressed the "left" squadmate button and realized that there's no portrait of the "right" squadmate. Curious, I pressed the right squadmate button but then my xbox crashed.

Is this common?

I've had it once on PC. Didn't crash though.

I had a situation that has happened a couple of times, where I would kill the 2nd harvester and never got the usual dialogue from the squadmates about what direction to go. I ended up walking all the way to Victus's location with no enemy interference and wasn't able to get the cutscene with Victus. What I did was exit out of the game and reload the mission. It solved the problem. This is playing on the ps3.

Oh, I thought you were talking about another bug. I've gotten the bug where those three husks don't spawn near the ladder going down near the end of the stage. :-/ Oh well, I always get stuck at the stupid Leviathan bug on PC anyways, but never on PS3, and that's saying something about the worst port for ME3. Luckily, there's hax for solving that. :-)

I've never gotten that throughout all of my playthroughs on PS3. My first time was on PC. Maybe it happens if you progress too fast after killing the second Harvester. They still haven't fixed that stupid Leviathan ladder bug where Shepard gets stuck while trying to go down the ladder. I'm just lucky I knew how to use console commands through keybinds before I started plaything through Leviathan on PC.

What ladder is that in Levaithan? I don't recall ever getting stuck on a ladder. On my current playthrough I plan on playing the dlc

It's one of the ladders that you take to go down. I believe it's the ladder you use before taking the elevator up to Ann Bryson. That ladder keeps getting me stuck, and I have to cheat to just drop down the building instead.

I've done that mission where no enemies spawned at all. I went all the way to Victus's position with no enemies at all , but the cut scene didn't play.

 

That was with Liara and Javik as team mates, I reloaded and the same thing. So I tried with Liara and Garrus and it worked.
