Depending on the severity of the betrayal the punishment will change.
Going by the 3 scenarios you wrote I'd guess it would go something like this
1. Unforgiving and swift death. That betrayal is far too severe to warrant a second chance, LI or not they are not going to get a second chance.
2. Their reason will dictate the punishment here. If they were forced to betray the Inquisitor then I'd probably more forgiving and probably just warn them that the next time no matter what they will get no mercy. If they're a good for nothing it's a quick death then move on, got bigger fish to fry.
3. One very stern warning that setting the Inquisitor up is not a healthy habit and should stop.
