I'm currently doing my first insanity playthrough in ME2. But I get really paranoid that maybe I'm not in Insanity or I did something wrong. Can someone just tell me if going to the gameplay options affects for me to not get the achievement?

As long as you don't change the difficulty option after leaving Earth, you should be fine.

We I play ME3 on insanity, I always make sure that the difficulty setting is on insanity as soon as I gain control of Shepard right after the reapers land in Vancouver. And as cap and gown mentioned above, do not change the difficulty at all once its set to insanity.

Mkay thanks.

To be honest, Insanity isn't as hard as I thought, so long as you know what you're doing.

If you know what you are doing all three games get easier. ;)

I had to reinstall my operating system, lost all bonus powers. I strongly suspect I lost my insanity status on my soldier play though. Anyone know how to confirm this? (other than finishing the game and seeing if you get the achievement)

If your old Bioware folder in My Documents is gone, you lost the achievements. (all of them). That's where everything is saved.

It's the only folder I'll copy to my new pc :P
