The previous games had the following structure:

 

- Difficulties: Casual > Normal > Hard > Nightmare

- Player cannot carry character to one another

- story mode is a completely singleplayer campaign 

- You chose one difficulty and play the whole game. For a new difficulty, make a new character. And nothing from previous playthroughs follows you.

- When story is complete (or a little bit before) you can do your unfinished business, and since the game have no respawn, the game is over.

 

Inquisition follow the same structure?

I think he means can't use the same character for a different play through

when you finish "normal" that character cannot start hard or nightmare with his current level, gear or anything, a new character must be created.

Far as we know the game will have the same difficulties though its been said it will be harder than Origns

I think your talking about New Game Plus and I think its been all but confirmed there isn't one

Yes the story mode is all single player, multiplayer will have no effect on SP

I think this is the same question as your 2nd one? 

Once you finish the story in Origins and DA 2 you can't do any unfinished business, the games end. Only things you can do in DA 2 after the main game is over   is DLC if you didn't play it during the game. It has been confirmed that in Inquisition you can keep playing once the main story is finished.

Actually, you can change difficulty setting during your playthrough - at least, in DA II

 
The story mode - yes, the previous games had only single player, so it's safe to say story mode = single player. DA: Inquisitior has multiplayer, but I don't know if it has some sort of story. I had assumed that multiplayer characters are the agents of the Inquisition who are in a kind of a spin-off without an actual story. 
 
When the story is complete in the Inquisition, you can continue playing. It had been confirmed. We don't know if it's possible even if you got the worst ending. But it's possible. I suppose it also means there is no New Game+.

the game is completely open world with a few loadings (like skyrim) or is something like origins with places to go in a map (you cannot walk from a town to other town)?

Level cap: 25-30

It's Origins world (with some areas inaccessible and unlocked later), not Skyrim open world.

I believe they've said we'll be able to change the difficulty at any point during the game.

DAO and DA2 didn't have this one: "You chose one difficulty and play the whole game. For a new difficulty, make a new character."

 

I changed a lot of difficulty during my games. If I get tired at some fight I change it to easy to make pass that. If fights are too boring I change them to hard. I really like this feature cause it means I won't get stuck anywhere XD

The game will feature much larger hub area to explore that DAO or 2 did but the will still be hub areas not fully open world like Skyrim. Some parts of the hub areas will be inaccessible until we fulfill certain criteria such as having agents cut off a source of poison gas filling a canyon, for example.

Don't know where the OP is getting his information from, but...

 

You can change difficulty level on the fly ANY TIME during the previous games. If you were unaware of this, well....LOL

 

Once the main campaign is wrapped up (in DAO and DA2), anything else you left undone is lost. You can't carry on doing side quests, only separate DLC campaigns if you have them.

This will change in DAI, however. You will be able to keep playing and doing side quests and stuff after the main story is done.
