For example, when I'm in my rover or the Normandy, the background noise
of the ship and the rover when it drives drowns out almost all of the
other sounds of the game such as speech, the machine gun turret, etc.
This is pretty annoying since All I can hear is the ship and not
the dialogue. Toying with the sound levels in the options does nothing
to help. It definitely isn't normal. Has anyone else come across this and is there any way to fix it?

not sure what it is but EXACT same issue.
Make sure you have your sound output set to 'software' ('hardware' is only supported on the creative soundblaster card... uber ghey) If you audio cuts out more than usual, usual meaning here and there is normal but if every cutscene get cut off and especially during combat guns make .001 second sound effects make sure you update your drivers. It really ruins the experience but is fixable... ish... mass effect is a pretty buggy game sound wise especially.

Wasn't there a codex fix for this, way back in 08?
For the life of me I can't recall it though.

Definitely a driver related issue for your sound chip.
