Just as the title says, if you import you save data from Origins that had your Warden sacrifice themselves to kill the Archdemon, when you start the Awakening DLC they will be alive again, with no explanation whatsoever. Just a huge plot hole.

It's not exactly a plot hole, but an engine limitation.

The official word is that Bioware couldn't figure out a way to import main campaign flags without also importing your character, hence why if you pick an Orlesian Warden you're stuck with default flags.

Bioware could have prevented Ultimate Sacrifice characters from being imported, but didn't want to. I suppose it felt too much like penalising people, and someone made a call (the right one, I think) that it was better to have the lore look a little wonky than annoy US players by completely ignoring all their DA:O choices.

If they'd prevented US importing, and those players wanted their flags to still carry forward (or, hell, they decided they wanted to keep playing with their Warden after all) those players would have had no choice but to go through the entire last section of the game again, way back from the Dark Ritual point -- and you can  bet not everyone had a hard save back there.

So they let people import US Wardens, and suggested people consider them as retroactive Dark Ritual Wardens if they want it to make sense.

Seriously, I think that's a better decision than being a hard-ass forcing people to play swathes of DA:O all over again or make an Orlesian with no carry-over flags.

Originally Bioware's answer for it came down to "Head canon the reason if you want to play your Warden."  ^_^

Canonically, she did not. I am quite certain that if that did happen, that was a mod.

I had also just installed Awakening. So, I had a few mods installed too. I'll see if I can make it happen again.
