Will it be making an appearance? Seems like it was an important plot-device in DA2 that was mentioned once or twice but not brought up again.

I'm sure at the least Varric will mention it in conversation. Wouldn't be surprised if we see a red lyrium statue of knight commander Meredith.

With what we've seen of the stuff, that statue is likely to come alive. Creepy!

Well red lyrium seems like it's at least going to be a bit of a problem. The idol in particular...it'll probably be mentioned, but no more than that.

Given the effects it has on the mind, I expect that allowing Varric to keep the Idol shard will lead to him being a completely different companion with 100% distinct lines.

The idol itself won't be making an appearance--it was turned into Meredith's sword.

 

As for the red lyrium statue that was Commander Stannard...here's a chilling thought:  we see red lyrium growing hand-over-foot on the surface, but where do you think the free-growing stuff came from?  The Carta might be responsible for some of it, but what if the Red Templars...cannibalized Meredith's remains?  Maybe that's how they started using red lyrium in the first place.  Not all templars would have a ready supply of lyrium on hand after breaking away from the Chantry, and you can only get the smuggled stuff from the Carta.

 

When people get desperate, they sometimes do horrific things.

I'm willing to bet that the primeval dwarves worshipped the Elder One and that the idol depicted him, so it'll probably come up.
